We're looking for an experienced Registered Nurse to join our team as a Nurse Case Manager, focusing on field-based supervision.

    Position Overview:
    The Nurse Case Manager conducts in-home supervisory visits to ensure services meet state regulations and agency standards. This role supports caregivers and nurses in the field, evaluates care delivery, and ensures client safety and satisfaction. Minimal office time is required — most work is completed in the field.

    Responsibilities:
    Perform in-home supervisory visits for in-home aides (PCAs and CNAs).
    Assess client needs and update care plans as necessary.
    Coach and mentor field staff to ensure quality and compliance.
    Document supervisory and case review visits per state and agency requirements.
    Respond to PRN assignments for urgent assessments or visits.

    Qualifications:
    Current RN license in North Carolina (unencumbered).
    At least 2 years nursing experience; 1 year in home care or community nursing preferred.
    Strong knowledge of NC home care regulations.
    Reliable transportation and valid driver’s license.
    Ability to travel extensively within service area.
    Comfort working in varied home environments.
    Proficiency with home care Electronic Health Records, especially WellSky, preferred.

    Schedule & Pay:
    PRN – flexible scheduling based on agency needs.
