We’re seeking skilled and caringLicensed Practical Nurses(LPNs) to join our Connecticut Home Health team.This traveling nurse role offers the autonomy of field work, the support of experienced clinical staff, and the opportunity to build meaningful one-on-one patient relationships.

Location:New Haven County- Hamden, CT

Schedule:

  • Monday-Friday, Weekend hours available

  • Full-Time, Part-time (24 hrs), On-Call, Holiday coverage

Qualifications:

  • ActiveConnecticut LPNlicense(required)

  • Minimum 6 months of clinical experience(hospital, LTC, SNF, rehab, med-surg, or similar)

  • Valid driver’s license, reliable vehicle, and can travel to visit clients

  • Strong assessment, communication, and documentation skills

  • Spanish speaking is a plus!

  • Home health experience is a plus!

Responsibilities:

  • Provide skilled nursing visits in the patient’s home

  • Document vitals during visits and report changes in condition

  • Implement and update individualized care plans under RN supervision

  • Deliver patient and caregiver teaching

  • Assist with medication management and wound care

  • Communicate effectively with physicians, RNs, therapists, and the care team

  • Document all care accurately and in a timely manner

  • Maintain professional boundaries and promote patient safety at all times
