Job Summary
Health Care 4 All, Inc. is seeking a
Part-Time Clinical Manager to oversee and support the delivery of safe, high-quality home health services. The Clinical Manager will provide clinical supervision, coordinate patient care, monitor staff performance, and ensure compliance with applicable home health, Medicare, and Medicaid requirements.
Key Responsibilities
- Provide clinical oversight and supervision of nursing and field staff.
- Coordinate and monitor patient care services and individualized plans of care.
- Review patient assessments, clinical documentation, and care plans.
- Conduct patient assessments and home visits as needed.
- Ensure care is delivered according to physician orders, standards of practice, and regulatory requirements.
- Monitor patient progress and address changes in condition or care needs.
- Support staff with clinical guidance and problem-solving.
- Ensure compliance with Medicare, Medicaid, state regulations, and agency policies.
- Participate in quality assurance and clinical performance improvement activities.
- Participate in on-call coverage as assigned.
Qualifications- Current, unrestricted RN license in the state of employment.
- Minimum 1 years of home health/home care experience.
- Minimum 1 year of clinical supervisory or leadership experience.
- Strong clinical assessment, leadership, and organizational skills.
- Knowledge of Medicare and Medicaid home health requirements.
- Experience with geriatric and/or pediatric patients preferred.
- Valid driver's license and reliable transportation.
- Current CPR/First Aid certification.
- Bilingual in English and Spanish preferred.
