Job Summary


Health Care 4 All, Inc. is seeking a Part-Time Clinical Manager to oversee and support the delivery of safe, high-quality home health services. The Clinical Manager will provide clinical supervision, coordinate patient care, monitor staff performance, and ensure compliance with applicable home health, Medicare, and Medicaid requirements.


Key Responsibilities


  • Provide clinical oversight and supervision of nursing and field staff.
  • Coordinate and monitor patient care services and individualized plans of care.
  • Review patient assessments, clinical documentation, and care plans.
  • Conduct patient assessments and home visits as needed.
  • Ensure care is delivered according to physician orders, standards of practice, and regulatory requirements.
  • Monitor patient progress and address changes in condition or care needs.
  • Support staff with clinical guidance and problem-solving.
  • Ensure compliance with Medicare, Medicaid, state regulations, and agency policies.
  • Participate in quality assurance and clinical performance improvement activities.
  • Participate in on-call coverage as assigned.
Qualifications


  • Current, unrestricted RN license in the state of employment.
  • Minimum 1 years of home health/home care experience.
  • Minimum 1 year of clinical supervisory or leadership experience.
  • Strong clinical assessment, leadership, and organizational skills.
  • Knowledge of Medicare and Medicaid home health requirements.
  • Experience with geriatric and/or pediatric patients preferred.
  • Valid driver's license and reliable transportation.
  • Current CPR/First Aid certification.
  • Bilingual in English and Spanish preferred.
