    Description

    Essential Duties and Responsibilities

    • Provides quality and safe delivery of services to patients in their homes as assigned and supervised by professional nursing staff according to the developed plan of care for each patient.
    • Assists/provides patients with personal hygiene, dressing, bathing, and other daily living tasks, homemaking services, nutrition through meal preparation, teaching and emotional support.
    • Assists/provides environmental maintenance and management which are essential to the patient’s health care at home.
    • Performs basic health care services for patients, including checking vital signs or administering prescription medication.
    • Assists patients to access specific preventive services as directed by the professional nurse
    • Informs individuals of services available and their benefits.
    • Making transportation arrangements as needed.
    • Works with a nurse, personal care aide, CNA, nurse aid, nursing assistant, and other caregivers and in-home care professionals.
    • Attends and participates in coordination of care meetings.
    • Reports any patient changes, concerns and incidents promptly.
    • Completes documentation, visit forms, and time studies within time limits specified.
    • Keeping up with in-service training.
    • Practices and behaves in a professional, safe and ethical manner at all times.
    • Serves as a patient advocate.
    • Maintains friendly and courteous interpersonal communication skills and a positive service attitude.
    • Participates in performance improvement and educational activities.
    • Works willingly and accepts assignments as given.
    • Attends all mandatory meetings and programs.
    • Participates in opportunities that enhance personal and professional growth.
    • Performs other duties as necessary or assigned.
    • Complies with the Corporate Compliance Policy and all laws, regulations, and Standards of Conduct relating to the position and agrees to report any suspected violations of law or Standards of Conduct.
    • Complies with all applicable state and federal regulations and JCAHO standards.
    • Maintains a safe work environment using safe work practices, utilizing standard precautions and complying with hazardous waste procedures at all times as described in St. Anthony's Safety Manual.
    • Maintains the confidentiality of patient, resident, employee and organizational information.

    Qualifications

    Education and/or Experience

    • High school diploma, GED, or similar certification required.
    • Certified Nursing Assistant (CNA) preferred.
    • One to three months of related experience and/or training preferred.

     

    Education and/or Experience
    High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.

     

    Position Requirements, Licenses, Certifications, Registrations

    • BLS (Basic Life Support) certification through the American Heart Association required. New hires may obtain certification during onboarding with organizational support.
    • Valid driver license.
    • Completion of MVR prior to hire and annually with an Acceptable grading criteria.
    • Proof of insurance upon hire and annually for personal vehicles used for company business (recommended minimum of $100,000 for property damage and $300,000 for bodily injury).
    • Excellent communication and interpersonal skills.
    • Ability to build positive relationships with different types of people, including residents, patients, employees, and members of the health care team.
    • Must possess effective time management skills.
    • Must be able to enter and function in non-handicapped accessible patient homes.

     

    Language Skills
    Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.  Ability to write routine reports and correspondence.  Ability to speak effectively before customers, patients, families and employees of the organization.
