Keystone Infectious Diseases is seeking a dedicated HIV Quality Management Coordinator / Medical Case Manager to join our team.
The HIV Quality Management Coordinator is responsible for leading and coordinating quality management and performance improvement activities for the Keystone Health HIV Program. This role develops, implements, and maintains the program's Quality Management Plan and ensures compliance with organizational standards and funding requirements through the creation and ongoing review of policies and procedures.
Responsibilities include participation in internal and external quality management committees, monitoring program performance, collecting and analyzing quality improvement data, and preparing reports for program leadership, Keystone Health, and funding agencies. The Coordinator also collaborates with staff to support quality improvement initiatives and facilitates consumer involvement by incorporating patient feedback into program improvement efforts.
In addition, this position serves as a Medical Case Manager for individuals living with HIV/AIDS. The Medical Case Manager coordinates medical and supportive services to promote continuity of care and positive health outcomes. Working closely with patients and healthcare providers, the Case Manager helps identify barriers to HIV primary care, supports adherence to treatment plans, and develops strategies to address patient needs. Services are provided through both in-person and remote interactions in office and community-based settings.
Candidates must meet one of the following qualifications:
Previous experience developing and implementing a quality management program is preferred. Candidates must be able to complete all required training and competency requirements.
