    Description

    Sage Group HealthCare Staffing is seeking a Home Health RN for 13-week travel/contract positions in Anchorage, AK. We are accepting both local and traveler candidates.

    Position Details:

    Position Name: Registered Nurse Home Health

    Location: Anchorage, AK

    Contract Length: 13 weeks

    Pay Rate: $3280 total weekly gross - $82/hour ($34/hour W-2 taxable , $48/hour non taxable stipend)

    Unit: Home Health

    Shifts available: Days

    Shift timings: 8 Hours; Days, 8:00 am - 5:00 pm 

    Schedule: 40 hours/week guaranteed, 5 shifts per week, Mon-Fri, No weekend or On-call

    Position Requirements:
    • Registered Nurse: 2 years - Required
    • Home Health RN experience: 1 year within the last 3 years - Required
    • EMR: Home Care Home Base (HCHB) charting
    • Must have valid Driver’s License and Personal Auto Insurance

    Education, License and Certification Requirements:

    • ASN or an equivalent – Required
    • BSN or an equivalent – Highly preferred
    • RN License for State of the state applying or Compact if applicable -  Required
    • BLS Certification from AHA or American Red Cross - Required

    Job Description:

    • Provide and coordinate care for assigned home health patients
    • Develop and implement nursing care plans for all all types of visits including Start of Care, ROC, Revisits, etc.,.
    • Support patient recovery.
    • Provide clinical oversight to interdisciplinary
    • Provide nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
    • Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
    • Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
    • Notify physicians of significant changes in status
    • Document and complete charting for all patient visits within 24 hours.
    • Territories to cover: Municipality of Anchorage
