Hourly Wage: $23.91 ~ $29.27
NATURE OF WORK
This position serves as a High Lethality Case Manager at the Family Peace Center (FPC). Through the use of an evidence-based danger assessment will assess the needs of clients and their family unit and arrange or provide a range of direct individual services to meet the immediate and long term needs of clients.
DISTINGUISHING FEATURES
The High Lethality Case Manager will dedicate 100% of their time to undertaking assessment, planning, advocacy and linking with long term case management, rehabilitation and support services that specifically address the needs of FPC clients who are high lethality.
ESSENTIAL FUNCTIONS (These essential duties are only illustrative.)
Provides long term case management to FPC clients while adhering to trauma informed and survivor driven principles and policies of the FPC; treating all clients, staff and FPC partner agency personnel with kindness, dignity and respect.
Engages in activities and practices addressing self-care and vicarious trauma.
Ensures the confidentiality and information sharing process is followed.
Participates in training and regular debriefs with the High Lethality Project Manager.
Leads and participates in multi-disciplinary team efforts connected to lethality prevention.
Provides feedback to the FPC Executive Director about gaps in service provision and identifies partners or people who need to be at the FPC in order to fulfill client multi-victimization needs.
Works with clients to develop and maintain relevant and meaningful case empowerment plans which include addressing employment, wellness, health, housing advocacy, public benefits, counseling and court accompaniment support.
Meets regularly with clients to support plan accomplishment.
Refers clients to other agencies and resources, advocate directly with other agencies as needed to help client access necessary services and accompany clients to services and appointments to support the successful implementation of the case management plan.
Ensures clients have an understanding and knowledge of resource options and facilitate connections and enrollment in those resources and mediate on behalf of residential clients when appropriate.
Completes all other tasks and duties as assigned and required by the Executive Director of the Mayor's Office of Domestic Violence and Community Violence Prevention, Client Services/Project Manager and High Lethality Project Manager.
SUPERVISION RECEIVED
Works under the general and day-to-day supervision of the High Lethality Project Manager and at the direction of the direction of the Client Services Manager, who reviews work for effectiveness, timeliness, and for overall results achieved.

This position does not require supervisory duties.
WORKING CONDITIONS & PHYSICAL DEMANDS
Work is performed primarily in a variety of settings, including office and various locations to meet client needs of support and advocacy. Work activities occur in a fast paced, high demand environment at times.
SUCCESS FACTORS (KSAs)
Thorough knowledge of domestic violence and human trafficking prevention efforts, including best practice prevention efforts and strategies.
Comfortable working in a collaborative manner in a fast-paced environment with multiple organizations. Sensitivity to the concerns of partner organizations, willingness to receive and use feedback in program design, interpersonal interactions and policy development.
Ability to express ideas effectively orally and in writing, to conduct research, and to prepare clear and concise reports.
Ability to establish and maintain effective working relationships with city employees, other government agencies, law enforcement, the general public, and community organizations.
Skill in the use and care of a personal computer.
Ability to ask for help and support.
Ability to treat clients, staff and partner agencies with kindness, dignity and respect in challenging circumstances and conditions.
Strong communication and interpersonal skills.
EDUCATION, TRAINING & EXPERIENCE
Graduation from an accredited college or university with a Bachelor Degree in human services, social services or the equivalent, and one-year experience in case management, 40 hour ICADV Domestic Violence Training and other training in confidentiality and mandated reporting and have knowledge of the Family Justice Center movement and the purpose of this model and service delivery. Knowledge of best practice interventions for persons in trauma of domestic or sexual violence, child or elder abuse or human trafficking. Experience with computer programs, data collection desired. Persons with lived experience with domestic or sexual violence, child abuse or human trafficking preferred. Any satisfactory equivalent combination of education, experience and training which ensures the ability to perform the work may be substituted for the required experience.
NECESSARY SPECIAL REQUIREMENTS
Possession of a valid Illinois driver's license.
40 Hour ICADV Domestic Violence training (may be completed within 30 days of hire)
Residency Requirement:
All employees (except Police and Fire) shall reside anywhere in Winnebago County or anywhere within fifteen (15) miles of the Rockford City Hall within six (6) months of completion of their introductory period, department heads and City Administrator shall live within the municipal boundaries of the City of Rockford within six (6) months of their completion of their introductory period.
