Responsibilities:
- Performs professional nursing services requiring the application of the biological, physical and social science and nursing skills in the care of the sick, the prevention of disease, and the conservation of health.
- Works in an interdisciplinary relationship with other members of the corporation's staff to provide coordinated Home Health services to individuals and their families in accordance with a Plan of Care established with the medical provider, patient and family.
- Assumes primary case management functions for assigned patients.

- Completes and maintains all competencies and trainings as required for role.
- Graduate of an accredited school of Nursing
Experience
Certification and Licensure
