Heart Failure Program Coordinator

Job Summary

The Heart Failure Program Coordinator:

• Coordinates and facilitates all components of the heart failure program.
• Directs the program's Quality Improvement components required to maintain accreditation standards and is responsible to maintain effective lines of communication between appropriate accreditation agencies.
• Ensures that the programming is patient-centered, effective, and efficient.
• Efforts are directed at optimizing the care received by the patient.
• Ensures integration of other disease programming as appropriate to address chronic comorbid conditions.
• Promotes collaborative care practices and participates in establishing partnerships with the physicians and the entities of the Reading Health System to provide identified related services.

Work Schedule: M-F, 7:00am-3:30pm

Qualifications

Education

• 4 year Bachelor''s Degree- OR- combination of relevant education & experience may be considered in lieu of Bachelor''s Degree

Certification and Licensure

• PA Registered Nurse License
• Specialty Certification Required within 6 Months of Hire
• Cardiac experience preferred
