HEALTHY START CARE COORDINATOR | FULL-TIME | MISSION-DRIVEN CAREER
At Children’s Home Society of Florida (CHS), we believe every child deserves the strongest possible start in life. As a Healthy Start Care Coordinator, you will help expectant parents, infants, and young children access the support, education, and resources needed to improve health outcomes and build brighter futures.
The earliest years of a child’s life can shape everything that follows. In this role, you will partner with families during pregnancy and early childhood, helping them navigate challenges, strengthen parenting skills, and connect to essential healthcare and community resources.
Your work may include:
• Conducting family screenings and assessments to identify needs and strengths.
• Developing individualized care plans and family support goals.
• Providing home visits, education, screenings, and ongoing family support.
• Teaching topics such as healthy pregnancy, newborn care, breastfeeding, nutrition, child development, safe sleep, and positive parenting.
• Connecting families to healthcare providers, transportation, and community resources.
• Supporting healthy parent-child relationships and developmental growth.
• Maintaining accurate documentation and case records.
Education & Experience
• High school diploma or equivalent required.
• Bachelor’s degree in Social Work, Health Education, Human Services, or related field preferred; Registered Nurse licensure preferred.
• Two years of experience in social services, healthcare, public health, or a related field preferred.
• Direct client-care experience preferred.
Additional Requirements
• Florida Driver’s License and reliable insured transportation required.
Knowledge & Skills
• Knowledge of pregnancy health, infant health, and child development.
• Strong communication, organization, documentation, and relationship-building skills.

At CHS, your support can help a parent feel confident, help a baby get a healthy start, and help a family build a brighter future. Bring your compassion and expertise to work that matters every day.
***The bonus will be paid in two installments with the first being after successful completion of your 90-day orientation period and the second installment being after your 12 months of service. This sign-on bonus is taxable; all regular payroll taxes will be withheld. In the event that you leave CHS within 12 months of your date of hire, you will be responsible for reimbursing CHS for the sign-on bonus at a pro-rated amount.
That commitment represents CHS’s focus on recognizing each child’s potential, nurturing opportunity and creating lasting impact.
Bring your coordination skills, compassion and community mindset to CHS, and help create stronger connections to care for students and families.
