Job Summary
The Vandalia Health Network is a physician lead clinically integrated network (CIN) with a focus on quality and cost-effective health care.
This position is an exciting addition to our CIN which will provide disease specific dietary expertise and education to enrolled participants in Network Food Is Medicine programs. This position is critical in the success of assisting patients in managing chronic disease and lowering the cost of care while providing nutritional support to address food insecurity.
The ideal candidate would have the following competencies:
Good communication and analytical skills and ability to work with professional and non-professional personnel.
Technologically savvy and comfortable with telephonic communication and documentation.
Experience with disease specific dietary education and recommendation (Diabetes, Hypertension).

Patient Group Knowledge (Only applies to positions with direct patient contact)
The employee must possess/obtain (by the end of the orientation period) and demonstrate the knowledge and skills necessary to provide developmentally appropriate assessment, treatment or care as defined by the department's identified patient ages.
Specifically the employee must be able to demonstrate competency in: 1) ability to obtain and interpret information in terms of patient needs; 2) knowledge of growth and development; and 3) understanding of the range of treatment needed by the patients.
