The AmeriCorps Student Care Coordinator will serve as a liaison between the student client and community partners, including health care providers, school personnel, and child care providers. The member will primarily participate in health screenings. The member will follow up with the failed screenings to see if students need our services. The member will also appropriately document care coordination activities, which include student medical appointments.
