HealthLink Program Manager

Demonstrated grant-management experience, including interpreting funded scopes of work and budgets, coordinating deliverables, maintaining documentation, tracking outcomes and deadlines, and contributing to reports in partnership with Development and Finance. Minimum of three years of responsible experience in community-based social services, public health education, nonprofit programming, human services, care coordination, or a related setting, including responsibility for planning and executing programs.

Key Responsibilities

PROGRAM LEADERSHIP AND OVERSIGHT

• Lead all HealthLink operations, including Prescription Assistance (RxAssist), diabetes, A1C and blood pressure programs, nutrition and chronic disease education, senior voucher enrollment, and other health education programming.
• HealthLink education, medication assistance, grant deliverables, and community services are the primary focus of this role. The Doc-On-the-Spot clinic and visiting specialty clinics are sister and partner programs for which the manager provides administrative coordination and oversight; they are not the central focus of the position.
• Ensure compliance with organizational, funder, and partner documentation requirements and
  coordinate with licensed clinical partners regarding protected clinical records and clinical compliance
  responsibilities.
• Capture and track program data in the organizational database system. Use the data to monitor program reach, service quality, outcomes, grant performance, and needed program improvements.
• Provide oversight and coordination for the Doc-On-the-Spot clinic and all visiting specialty clinics. This
  responsibility is administrative and partnership-focused; it does not make the clinic the primary focus of the position or require the manager to provide clinical care.

SUPERVISION AND STAFF MANAGEMENT

• Supervise the Doc-On-the-Spot staff and all HealthLink program staff, including the Medical Advocate(s).
• Provide training, leadership, mentorship, scheduling, evaluations, and performance oversight.

HEALTHCARE PARTNERSHIPS AND COORDINATION

• Serve as the primary liaison to the Lions Eye Clinic, Doc-On-the-Spot, UAB partnerships, Hear Here, Five Horizons, the Area Agency on Aging, and additional healthcare partners.
• Hold quarterly coordination meetings with Doc-On-the-Spot, UAB, and partner providers.
• Provide onboarding and orientation for rotating physicians, podiatrists, specialists, and visiting clinical
  teams to ensure smooth integration and clear communication.

TRAINING AND EDUCATION

• Coordinate diabetes management classes, nutrition education, and chronic disease workshops. This includes assessing community needs; developing and maintaining an annual education calendar; selecting appropriate curricula, presenters, and partners; establishing measurable goals; coordinating
  logistics, outreach, and participant recruitment; overseeing execution; documenting attendance and outcomes; and evaluating and improving each program.
• Ensure HealthLink staff understand each program model, referral pathway, grant requirement, documentation standard, and their role in successful execution.
• Demonstrate willingness to become certified in diabetes education and/or obtain other certifications
  deemed appropriate by management.

FISCAL AND ADMINISTRATIVE RESPONSIBILITIES

• Manage the program budget and purchasing. Monitor budget-to-actual spending, funding restrictions, procurement documentation, and alignment of expenditures with approved program activities in collaboration with Finance and leadership. Gather data and complete reporting on weekly and bi-weekly basis utilizing the organizations data base platforms.
• Provide accurate data to the Development office for grant reporting. Manage the programmatic side of
  grants and contracts in partnership with Development, Finance, and leadership. Review and understand approved proposals, scopes of work, budgets, deliverables, performance measures, and deadlines; translate commitments into implementation plans and calendars; monitor activities, outcomes, documentation, and spending; maintain organized, audit-ready grant files; identify variances or compliance risks early; contribute narrative, data, impact stories, and supporting documentation; and assist with amendments, renewals, site visits, and funder follow-up. This is not the organization’s primary grant-writing role, but it requires active grant management and collaboration with the Development team.

WORK EXPECTATIONS

• Maintain professional relationships across partner agencies and clinical networks.
• Coordinate treatment teams and visiting clinical groups using the Doc-On-the-Spot clinic or other medical programs on site. This coordination includes schedules, space, communication, participant flow, referrals, and follow-up. Licensed clinical professionals retain responsibility for clinical assessment, diagnosis, treatment, and medical decision-making.
• Provide orientation, introductions, and facility walkthroughs for new clinical partners and their teams.
• Coordinate and support continuous coverage of the front desk area as needed, ensuring a welcoming and efficient environment.
• Perform basic administrative duties, including phone management, filing, communication support, mail-outs, and general office operations.
• Live within a reasonable commuting distance.
• Work occasional evenings or weekends for special events or clinical needs.
• Perform all other duties as assigned.

QUALIFICATIONS
Required

• Bachelor's degree in social work, human services, public health, health education, nonprofit management, community development, sociology, psychology, or a closely related field. A comparable combination of relevant education and substantial community-program experience may be considered.
• Current CPR certification or ability to obtain certification within 90 days of hire.
• Minimum of three years of responsible experience in community-based social services, public health education, nonprofit programming, human services, care coordination, or a related setting, including responsibility for planning and executing programs.
• Demonstrated ability to work effectively with healthcare providers and community partners, understand
  appropriate role boundaries, and connect participants with qualified clinical resources. Clinical expertise is not required.
• Experience supervising staff and establishing clear priorities, workflows, accountability, and performance expectations.
• Proficiency with case-management, program-data, or client-record databases; Microsoft Office or Google Workspace; spreadsheets; and accurate electronic documentation. Experience working with electronic health records is helpful but not required.
• Strong communication, organizational, and leadership skills. Must demonstrate strong follow-through, project management, sound judgment, attention to detail, and the ability to move programs from planning through implementation and evaluation.
• Demonstrated grant-management experience, including interpreting funded scopes of work and budgets, coordinating deliverables, maintaining documentation, tracking outcomes and deadlines, and contributing to reports in partnership with Development and Finance.

Preferred 

• Experience in nonprofit, social services, community health, public health, or other community-based settings.
• Familiarity with prescription assistance programs and chronic disease resources.
• Experience with health education curricula, community outreach, medication-assistance programs, benefits or resource navigation, continuous quality improvement, or grant-supported program development.
