With a reputation for exceptional client care, this team is seeking a Healthcare Intake & Referral Coordinator to help support referral management, admissions, insurance verification, and client onboarding.
Position Summary
This highly visible role serves as a central point of communication between referral sources, healthcare providers, clients, families, and internal care teams. The ideal candidate will have experience coordinating healthcare referrals, managing documentation, verifying insurance coverage, and ensuring a smooth admission process for new clients.
Key Responsibilities
- Coordinate incoming referrals and admissions
- Verify insurance eligibility and obtain required authorizations
- Manage client demographics, medical information, medications, and documentation within the EMR system
- Communicate with referral partners, healthcare providers, clients, and families
- Process medical orders, referrals, and related documentation
- Create and maintain client care plans and required records
- Track referral activity and client data using spreadsheets and internal systems
- Resolve client inquiries and concerns in a timely, professional manner
- Assist with scheduling and occasional on-call coverage
Qualifications
- Associate degree required; Bachelor's degree preferred
- Experience in healthcare intake, admissions, or referral coordination
- Medicare intake experience preferred
- Home health, hospice, or home care experience preferred
- Knowledge of insurance verification, prior authorizations, and payor types
- Familiarity with medical terminology
- EMR experience and strong computer skills
- CMA certification preferred
Position Details
- Schedule: Monday-Friday, standard business hours
- Location: 100% onsite in Woodbury, MN
- Employment Type: Temp-to-Hire
- Pay: $26-$29/hour
