In this hands-on role, you will serve as a key liaison between participants, healthcare providers, and internal nursing staff to ensure coordinated and effective care.Reporting to Health Services leadership, you will spend your time transporting participants to appointments, supporting them during medical visits, and ensuring accurate communication and documentation across care teams.
