Healthcare Advocate / Care Coordinator - Clergy Care

You will work alongside Meta Care’s team of pharmacists, nutritionists, social workers, and care coordinators, bringing additional expertise and support to each priest’s needs. Meta Care is seeking a compassionate, organized healthcare professional to support priests throughout the Diocese of Rockford through our Clergy Care program.

In this role, you will build meaningful relationships, help priests navigate healthcare and benefits, and follow through so they feel heard, supported, and connected to the resources they need.

 A nursing license is not required. We welcome candidates with experience in patient advocacy, care coordination, medical offices, healthcare navigation, or nursing.

A meaningful role with a team behind you
You will be a familiar face and dependable resource for the priests you serve—someone they can turn to with questions, concerns, or help navigating their healthcare. Through personal visits and regular check-ins, you will build relationships, understand their needs, and help make getting care easier.

You will work alongside Meta Care’s team of pharmacists, nutritionists, social workers, and care coordinators, bringing additional expertise and support to each priest’s needs. This is a non-clinical role centered on personal connection, advocacy, and thoughtful follow-through.

What you will do

• Build trusted relationships through regular outreach and visits with priests in their homes, parishes, hospitals, and other care settings.
• Help members find providers, arrange appointments, understand benefits, and navigate healthcare questions.
• Support priests during hospitalizations and following discharge by helping coordinate communication, resources, and follow-up needs.
• Identify unmet needs and connect members with appropriate healthcare, community, and Meta Care support services.
• Encourage preventive care and participation in available wellness programs.
• Work with Meta Care colleagues and designated diocesan contacts while protecting each member’s confidentiality.
• Maintain accurate documentation and follow through on outstanding needs.

What you will bring

• At least three years of healthcare experience, such as patient advocacy, care coordination, medical office support, or nursing.
• Strong listening and communication skills, with the ability to build trust and handle sensitive conversations with discretion.
• Excellent organization, attention to detail, and dependable follow-through.
• The ability to work independently while staying connected with a supportive team.
• Comfort using Microsoft Office and electronic documentation systems.
• A service-oriented approach and commitment to performing the role consistently with Catholic Social Teaching and the Ethical and Religious Directives for Catholic Health Care Services.
• Availability for in-person visits and travel throughout the assigned diocesan service area.

Successful completion of required background checks and screenings is required.

Compensation and benefits

• Annual salary of $55,000–$60,000.
