Summary Statement
The Health & Human Services Case Manager II is responsible for providing comprehensive case management services to individuals and families, including conducting assessments, developing service plans, coordinating resources, and ensuring timely access to essential supports. This position maintains accurate documentation, meets regulatory requirements, and collaborates with internal teams and community partners to promote client stability and well-being.
Because a significant portion of residents primarily speak Spanish, the Case Manager II provides bilingual communication to ensure equitable access to services, reduce language barriers, and support clear, compassionate interactions between residents and staff.

• Bachelors degree or higher in Behavioral or Social Science or related field. • Read, write, and speak Spanish fluently.

An employee in this class may participate as a member of a multi-disciplinary team to implement behavioral health services or treatment plans in accordance with Delaware Code Title 24.
Interviews
Client, family, or professional sources are interviewed to determine the clients needs for a variety of health and human services, which may include:
Develops a comprehensive, cost-effective service plan utilizing in-house sources, referrals, contracted providers, or institutions. Service plans may include:
Conducts ongoing monitoring and assessment of service delivery for optimum quality and efficiency, authorizes and recommends necessary changes and adjustments.
Maintains comprehensive case records, summaries, statistical, and narrative reports, and completes required forms.
Establishes effective working relationships with clients, family, community, and professional sources.
Provides guidance and relevant information to connect clients with needed services, which may include protective adoption and related services.
Provides information and case coordination on-site via email or telephone to individuals or groups.
Supplies information and responses to inquiries regarding services for clients, family, community, and government agencies.
May provide supportive casework to clients prior to, during, or after hospitalization based on physician diagnosis and recommended treatment.
Uses automated information systems and computer software to research, review, evaluate, and report on health-human service data.
Communicates and interacts effectively with people across cultures, ranges of ability, genders, ethnicities, ages, and races.
Performs other related duties as required.

This is a Casual, Seasonal position, which is a non-pension eligible position without healthcare and/or other benefits.
Conditions of Hire

DHSS does not provide employment-based sponsorship. All new hires are required to report to the Statewide New Employee Orientation (SNEO) on their first day of employment. Sessions are held at designated locations in Kent or New Castle County.
Pre-employment Drug Testing
Upon a conditional offer, you must undergo pre-employment drug testing as part of the hiring process.
Criminal Background Check
A satisfactory criminal background check is required as a condition of hire. The recruiting agency may require the applicant to pay for the criminal background check as part of the conditional offer of hire.
Upon a conditional offer, the applicant must undergo a pre-employment baseline two-step tuberculin skin test (TST) as part of the hiring process.
This position is a classification organized under an exclusive bargaining representative labor organization that has been elected by employees as their representative for collective bargaining and other applicable terms and conditions of employment in accordance with Title 29 Chapter 59 and Title 19 Chapter 13 and 16.
This position is covered by a collective bargaining agreement based on its individual terms.
