AccessHealth is a program to assist qualifying low-income adults without health insurance in Anderson County. AccessHealth brings together a network of healthcare providers and community agencies to ensure that those in underserved communities have access to high quality of care in the right place at the right time. The role of the Health Coach is to establish and maintain collaborative relationships with physicians, other health care providers, patients, and their families, to achieve desired patient outcomes throughout the continuum of care. The Health Coach is to conduct outreach on specific populations as identified by the Healthy Outcomes Plan or Population Health initiatives; implement strategies to promote, maintain and improve individual health; and reach defined or targeted outcomes. The Health Coach provides care management in a variety of settings to include inpatient/bedside, medical homes/PCP, community, video visits and/or home visits. Care management will include a variety of issues, both clinical and nonclinical. Duties Responsibilities Plans, prepares and implements processes and procedures for effective patient outreach and care coordination across the continuum. Provides social support by conducting health assessments, acting as a patient advocate and coordinating participation in health and social services programs or providing information on available resources. Assists the patient in utilizing appropriate and available resources when necessary. Coordinates care with physician practice personnel, AnMed Departments, and/or internal/external resources as identified. Assist patients in compliance with health screenings, maintaining/keeping office appointments and compliance with a course of therapy for identified chronic conditions. Assists in the transition of care from inpatient or ED visit, communicating with both the patient and family, and provider post-discharge. Qualifications High school graduate or possess a GED. LPN with current SC license or CMA certification by American Association of Medical Assistants (AAMA), National Center for Competency Testing (NCCT), National Healthcare Association (NHA), or American Medical Technologist (AMT). Any combination of three years health/social service experience and/or education. Must have a valid drivers license and reliable transportation.
