Health Center Nurse (HCN) Job Summary

Under the general administrative direction of the District Nurse Administrator, this position serves as the Health Center Nurse and supervisor of medical assistants. The HCN leads, directs and supports the clinical/nursing operations at the Health Centers. The HCN is directly responsible for the medical assistants, within the health centers. The HCN will work closely with the providers and other Health Center leaders to assure direct patient care is provided to all medical customers. The HCN is directly involved in the coordination and implementation of clinical, ancillary, and business services within the health center in a way that meets or exceeds standards while providing excellent patient experience across all services.

Job Duties

Principal Duties and Responsibilities:

  • Responsible for the daily management and staffing of medical assistants to provider ratios.
  • Monitor and evaluate staff performance.
  • Physically present in clinic and act as leader in charge for back office and clinical services.
  • Participate in clinical activities to assess and model competencies.
  • Promote efficiency between clinics by ensuring staff are performing daily huddles to plan for patient care.
  • Organize and prioritize clinical responsibilities of medical assistants.
  • Recognize leadership potential in staff and encourage growth.
  • Facilitate compliance with required educational offerings.
  • Hold staff accountable for strategic goals and project outcomes.
  • Gather data to support change.
  • Set expectations for policy and procedure adherence.
  • Create a culture of safety within back office/clinical staff.
  • Responsible for setting expectations for clinical Environment of Care.
  • Report day to day activities, volume and staffing benchmarks and any clinical concerns.
  • Serve as a local clinic leader in the absence of others.
  • Ensure medical assistants work at top of scope and practice within limits of their credentials.
  • Manage transitional change. Monitor institutional clinical Safety concerns and analyze for trends and targeted interventions.
  • Is the expert in practice issues for designated health centers.
  • Provide education for specialty areas; such as Classes for Asthma, Diabetes, Wellness, and so forth, and assist provider in monitoring within the scope of license.
  • Analyze weaknesses and areas of growth to create effective teams.
  • Oversee The Joint Commission visit preparedness at Health Center.
  • Conduct audits to ensure culture of readiness at Health Center.
  • Represent department at institutional regulatory meetings.
  • Implement programs that develop service excellence skills.
  • Serve as a general resource for clinical EHR functionality and workflows.
  • Monitor and audit adherence to clinical protocol workflows.
  • Involved in the adequate training and development of medical assistants.
  • Work closely with providers to enhance staff EHR functional skills.
  • Display willingness to manage all patient needs within the clinic environment wherever possible.
  • Maximize provider efficiency through execution of policy and protocols for medical assistant staff.
  • Implement novel approaches to care.
  • Conservatively manage clinical operations to stay on budget.
  • Establish relationships with other leaders in clinic to leverage collective knowledge and capital.
  • Collaborate with other health care providers, Chiefs of Medical Affairs, Director of Nursing, District Nurse Administrators, and Medical Director regarding clinic and patient needs.
  • Collaborate in the organization's continuous Performance Improvement Program (PIP).
  • Collaborate with all clinical staff to achieve clinical quality indicators.
  • Train staff and monitor patient care plans in conjunction with Care Managers to achieve patient centered goals toward improved health independence.
  • Assist in reporting and collaborating with health plan contacts to ensure follow through and maintenance of program.
  • Provide hands on treatment as necessary and within the scope of RN license to patients entering clinic including starting of IV's, performance of blood draws, administration of medication etc.

Other Responsibilities:

  • Participate in Patient Centered Health Home team meetings and quality improvement initiatives.
  • Provide health and disease patient education, including leading group office visits.
  • Support patient self-management of disease and behavior modification interventions.
  • Ensure coordination of continuity of patient care with external health organizations and facilities, including the process hospital admission and discharge and referrals from primary care provider to specialty care provider.
  • Ensure coordination of continuity of patient care with patients and families following hospital admission, discharge, and ER visits.
  • Ensure managing of high risk patient care; including management of patients with multiple co-morbidities or high risk for readmission to a hospital setting, including a registry.
  • Ensure patient medication management based upon standing orders and protocols.
  • Participate in community functions as appropriate.
  • Assume other responsibilities as required.

Other Duties:

  • HIPAA compliance - responsible for maintaining abreast of and in compliance with all HIPAA regulations and requirements. Treats all member information as confidential.
  • Compliance - Ensure compliance with all local, state and federal regulations.
  • QA/QI - Participate in QA/QI activities and contribute towards the overall performance improvement of the organization.
  • IT - Required to learn and use the Electronic Health Record and Practice Electronic System and its components. As required by the job functions and highlighted in the Policies and Procedures.
  • All employees will participate in Patient Centered Health Home Model at Omni Family Health.

Qualifications, Education, and Experience:

  • Licensed Registered Nurse from an accredited Nursing Program.
  • Licensed Vocational Nurse will be considered if they meet the qualifications and experience as outlined on this job description.
  • 1 year of management or supervisory experience in a clinical environment preferred.
  • Minimum 2 years' experience in outpatient operations experience as a nurse preferred.
  • Experience in a federally qualified health center or rural (FQHC) health center (clinical) setting preferred.
  • Proficient computer skills, including Microsoft Office (specifically Work and Excel)
  • Highly organized and well-developed oral and written communication skills.
  • Knowledgeable through practical experience and/or training in the areas of:

  • Bilingual in English and Spanish preferred, but not required.
  • Must have current CPR card.
  • Must be willing to work at any Omni Family Health locations and be amenable to work weekends, if so needed.
  • Must provide a safe, operational vehicle for transportation.
