Health Care Occupational & Case Manager
Position Type: Full-Time / Hybrid
In-Office Days: Monday, Tuesday, and Thursday / 6:45 AM – 3:30 PM (40hr/Wk)
Pay: 43/hr (Paid Weekly)

Position Overview
We are seeking a qualified Health Care Occupational & Case Manager to coordinate and manage care for employees with complex medical conditions, disabilities, work-related injuries, or occupational health needs. This position works closely with employees, healthcare providers, insurance carriers, and internal teams to facilitate appropriate care, support recovery, and ensure compliance with applicable medical, regulatory, and workers' compensation requirements.
The ideal candidate will have experience in occupational health nursing, case management, workers' compensation, disability management, or a related healthcare field, along with strong clinical judgment, communication, and organizational skills.
Key Responsibilities
- Assess employees' medical, psychological, occupational, and social needs
- Develop and coordinate individualized case management and care plans
- Coordinate medical services, treatments, therapies, consultations, and follow-up care
- Monitor employee progress and modify care plans as appropriate
- Provide education and support to employees, families, and caregivers
- Facilitate communication between employees, healthcare providers, insurance companies, and internal stakeholders
- Support management of workers' compensation, disability, limited-duty, and occupational health cases
- Assist with return-to-work and work-restriction coordination
- Apply knowledge of regulatory requirements, healthcare plans, and external healthcare resources
- Document assessments, care plans, communications, and case activities accurately and timely
- Identify barriers to care and develop appropriate solutions or referrals
- Evaluate the effectiveness of case management interventions and recommend improvements
- Maintain confidentiality and comply with applicable healthcare and regulatory requirements
- Contribute to a collaborative team environment and support departmental goals
Required Qualifications
- Registered Nurse (RN), Nurse Practitioner (NP), or Physician Assistant (PA) licensed in the state of IL
- Minimum of 2 years of occupational health nursing or related healthcare experience, depending on experience and work history
- Knowledge and understanding of case management principles
- Experience or knowledge related to:
- Workers' compensation
- Disability management
- Limited-duty/work restrictions
- Workplace safety
- Industrial hygiene
- Healthcare plans
- Regulatory medical requirements
- External healthcare resources
- Strong clinical assessment and critical-thinking skills
- Excellent verbal and written communication skills
- Strong organizational and time-management abilities
- Ability to work independently while contributing effectively to a team environment
Preferred Qualifications
- Experience in occupational health or employee health
- Experience managing workers' compensation or disability cases
- Certified Case Manager (CCM) certification or related case management credential
- Experience coordinating return-to-work programs and workplace accommodations
Work Environment
This is a hybrid position requiring in-office work on Monday, Tuesday, and Thursday from 6:45 AM to 3:30 PM. The position requires frequent communication and collaboration with employees, healthcare providers, insurance representatives, and internal teams. The ability to effectively manage cases both independently and as part of a multidisciplinary team is essential.
