Health Care Occupational & Case Manager. This is a

Pay Range: $37 - $38/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
Key Responsibilities:- Assessing patients' medical, psychological, and social needs.
- Developing individualized care plans in collaboration with healthcare providers and patients.
- Coordinating healthcare services, including medical treatments, therapies, and consultations.
- Monitoring patients' progress and adjusting care plans as needed.
- Providing education and support to patients and their families.
- Advocating for patients' needs and rights within the healthcare system.
- Communicating with healthcare providers, insurance companies, and other stakeholders to ensure coordinated care.
- Evaluating the effectiveness of care plans and making recommendations for improvement.
- Documenting patient assessments, care plans, and interactions.
- Keeping abreast of healthcare regulations, policies, and best practices related to case management.
- Health Care Occupational and Case Managers should have a background in nursing, social work, or a related field, along with experience in case management.
- They should have strong communication, organizational, and problem-solving skills.
- A compassionate and empathetic approach to patient care is essential.
- Depending on the specific role, certification as a Case Manager (CCM) or in a related field may be required or preferred.
- Schedule: 7am-3:30pm.
- Will report to location on Thursdays or as needed.
- On-call weekly rotation schedules every 4 weeks.
- Will be on call 24-hours for work-related injury, illness, or exposure reporting.
Key Requirements and Technology Experience:- Strong customer service, assessment & adaptability skills
- Critical care experience, occupational health, or case management experience
- Current BSL (Basic Life Support) Certification.
- Registered Nurse Degree: Bachelor of Science in Nursing or Master of Science Degree preferred
