Immediate need for a talented Health Care Occupational & Case Manager. This is a 12+ Months Contract opportunity with long-term potential and is located in Baltimore, MD (Hybrid). Please review the job description below and contact me ASAP if you are interested.

Pay Range: $37 - $38/hour.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
 
Key Responsibilities:
  • Assessing patients' medical, psychological, and social needs.
  • Developing individualized care plans in collaboration with healthcare providers and patients.
  • Coordinating healthcare services, including medical treatments, therapies, and consultations.
  • Monitoring patients' progress and adjusting care plans as needed.
  • Providing education and support to patients and their families.
  • Advocating for patients' needs and rights within the healthcare system.
  • Communicating with healthcare providers, insurance companies, and other stakeholders to ensure coordinated care.
  • Evaluating the effectiveness of care plans and making recommendations for improvement.
  • Documenting patient assessments, care plans, and interactions.
  • Keeping abreast of healthcare regulations, policies, and best practices related to case management.
  • Health Care Occupational and Case Managers should have a background in nursing, social work, or a related field, along with experience in case management. 
  • They should have strong communication, organizational, and problem-solving skills. 
  • A compassionate and empathetic approach to patient care is essential. 
  • Depending on the specific role, certification as a Case Manager (CCM) or in a related field may be required or preferred.
  • Schedule: 7am-3:30pm. 
  • Will report to location on Thursdays or as needed. 
  • On-call weekly rotation schedules every 4 weeks. 
  • Will be on call 24-hours for work-related injury, illness, or exposure reporting.
Key Requirements and Technology Experience:
  • Strong customer service, assessment & adaptability skills
  • Critical care experience, occupational health, or case management experience 
  • Current BSL (Basic Life Support) Certification.
  • Registered Nurse Degree: Bachelor of Science in Nursing or Master of Science Degree preferred
