    Overall Summary:

    Health Care Associate III (HCA III) provides customer care to all Bay Area Community Health (BACH) patients. The HCA III supports a variety of operational functions, which may include assisting with transportation and coordination of clinic resources, such as mobile or community-based services. The HCA III will communicate with care teams and patients using telephone, fax, patient portal, Microsoft Teams, and secure email tools. HCA III position also require competency in electronic health records. HCAs will focus on customer service, teamwork, quality, and timely deliverables. HCAs often interact with patients and provide services, such as but not limited to, registering patients to be seen on the mobile clinics; appointment confirmations; collection of patient co-payments, booking follow-up appointments, receiving inbound or making outbound calls, verifying insurance, and /processing medical records.

    Essential Responsibilities:

    Under the general direction of Program Supervisor, the Health Care Associate III performs a variety of complex, professional, analytical, and confidential services.

    Duties include:

    • Perform all duties for HCA I and II, plus:
    • Ability to assess patient health plan eligibility
    • Familiar with services offered within BACH
    • Liaison to external resources, as needed, for coordination of care
    • Recordkeeping of progress to ensure completion of care
    • Provide a variety of administrative duties
    • Work flexible hours which includes some evenings and weekends
    • Work at any of BACH locations, including clinics, mobiles, schools, encampments and patient appointments, as assigned
    • Assisting with coordination, preparation, and support of clinic operations in various settings, including mobile or off-site locations
    • Supporting transportation and logistical needs for clinic services, as assigned
    • Communicate stocking, maintenance, and cleaning needs, as appropriate
    • Calculate charges, collect patient payments, and balance monies with daily receipts per clinic protocol, as assigned
    • Assist patients with appointment scheduling, information requests and resource materials
    • Enters data into computer accurately and efficiently

    Secondary Responsibilities:

    • Attend workshops, training, and meetings as needed and as requested.
    • Perform special project research and data collection activities as requested; analyze and interpret data, and prepare reports
    • Assist with inventory management, including ordering and tracking supplies
    • Support general maintenance and upkeep of clinic equipment, vehicles, or operational resources, as assigned
    • Attends workshops, training, and meetings, as assigned
    • Assist BACH management with projects or tasks, as assigned
    • Perform other duties as assigned by supervisor
    • Provides clinic front desk coverage support when/where requested.
