    Health & Wellness Director – Lead an EPIC Wellness Team!

    CountryHouse Lincoln II | 84th & Pine Lake | Lincoln, NE | Full-Time

CountryHouse Lincoln II, an Agemark Senior Living community, is looking for a Health & Wellness Director to lead our wellness team and help residents live their best lives.

    What You’ll Do

    • Lead & Develop Your Team: Coach, support, and grow wellness team members while creating a positive, collaborative environment.

    • Be Present & Involved: Stay connected with residents and staff and provide hands-on support when needed.

    • Create Personalized Care Plans: Develop and maintain service plans based on each resident’s needs, assessments, and physician orders.

    • Build Strong Relationships: Partner with residents, families, team members, and healthcare providers to ensure clear communication and coordinated care.

    • Oversee Resident Care: Help ensure residents receive safe, compassionate, person-centered care every day.

    • Manage Medications: Oversee medication management practices and promote accuracy and safety.

    • Assess New Residents: Evaluate prospective residents and help determine how our community can best meet their needs.

    • Ensure Compliance: Maintain accurate documentation and ensure wellness practices meet state regulations and Agemark standards.

    • Support Safety & Wellness: Identify changes or risks, respond to emergencies, and implement proactive plans to support resident well-being.

    • Make a Positive Impact: Help create a community where residents feel known, respected, supported, and at home.

    What We’re Looking For

    • RN or LPN: Active Nebraska nursing license.

    • Senior Care Experience: At least 1 year of experience working with seniors.

    • Leadership Experience: At least 2 years of experience leading or managing a team.

    • Medication Knowledge: Strong understanding of medications, dosages, and potential side effects.

    • Strong Leadership Skills: A compassionate, dependable leader with excellent communication and problem-solving abilities.

    • Tech Savvy: Comfortable using software and electronic systems.

    • Flexibility: Availability to support the community as needed, including occasional evenings, weekends, and Manager on Duty responsibilities.
