Are you passionate about helping people find and maintain stable housing? The Permanent Supportive Housing Case Manager works with individuals and families who have experienced chronic homelessness and are living with disabling conditions. You'll help clients navigate the housing process from eligibility and housing search all the way through move-in and long-term housing stability.

This position follows a Housing First approach, meaning we believe everyone deserves a safe and stable place to call home. You'll build relationships with clients, understand their individual goals and needs, and provide the ongoing support they need to maintain housing and build greater stability and independence.

You'll also work closely with landlords, Housing Connect, community providers, and First Step House teams to address barriers and resolve issues before they put a client's housing at risk. What You'll Do

  • Help clients navigate eligibility, housing searches, applications, and move-in.
  • Complete needs assessments and create individualized case plans.
  • Provide ongoing case management and support to help clients maintain stable housing.
  • Connect clients with community resources related to healthcare, employment, education, benefits, and other needs.
  • Build positive relationships with landlords and help resolve housing concerns or conflicts.
  • Advocate for clients and respond quickly to issues that may put their housing or safety at risk.
  • Complete required documentation, including HMIS and Housing Connect records.
  • Collaborate with Housing Connect, community providers, landlords, and FSH teams to support client stability.
  • Maintain confidentiality and follow HIPAA, CFR 42, and other applicable requirements.

Required Qualifications

  • Case management experience required; housing or permanent supportive housing experience preferred.
  • Bachelor's degree in social work or a related field preferred.
  • Experience working with individuals experiencing homelessness, mental illness, or substance use preferred.

  • Able to build trusting relationships while maintaining professional boundaries.
  • Comfortable working independently, managing multiple priorities, and completing timely documentation.
  • Familiarity with HMIS, electronic databases, and Microsoft Office preferred.
  • A genuine commitment to Housing First, client choice, and helping people maintain long-term housing stability.
  • A willingness to approach clients and coworkers with an Outward Mindset and contribute to the mission of First Step House.
