Seeking an experienced BE/BC Family Medicine, Internal Medicine, or Geriatrics Physician to join its team. This position offers a mix of outpatient clinic and nursing home.

Position Highlights

• 2.5 days in an outpatient clinic, 2 days in nursing home care
• Collaborative team including a physician, nurse practitioner, and RN
• Built-in referral network from immediate care and specialty services
• Patient-Centered Medical Home and National Health Service Corps (NHSC) site
• System-wide EMR – EPIC
