Job Requirements

Education Requirements:

• Bachelor's degree in occupational therapy or related field

Certifications Requirements:

• BLS certification
• WI State License
• Valid Driver’s License with active auto insurance

Experience Requirements:

• Minimum of 3 years of occupational therapy experience
• Previous home health or acute care experience preferred
• Experience working with geriatric patient populations

Other Requirements:

• Ability to support within the interdisciplinary team, including RN and PT
• Strong evaluation and treatment skills focused on ADLs and adaptive equipment
• Ability to float to neighboring counties (Kenosha and Racine)
• Proficiency with EMR systems, specifically EPIC preferred
• Comfortable working in a hybrid onsite/remote environment

Job Responsibilities

• Collaborate with interdisciplinary teams including RN and PT to develop comprehensive patient care plans.
• Provide direct supervision and support to nursing staff, ensuring high standards of geriatric care.
• Evaluate patients’ functional status and develop tailored treatment strategies focused on ADLs and adaptive equipment.
• Maintain accurate documentation of patient progress and treatment activities using EPIC EMR.
• Support floating to neighboring counties as needed, ensuring continuity of care across locations.
