We are seeking an experienced and compassionate Full-Time Hospice RN Case Manager to serve patients and families in Fort Worth. This field-based position is ideal for a dependable nurse who is passionate about delivering outstanding end-of-life care with dignity, empathy, and clinical excellence.


As an RN Case Manager, you will oversee and coordinate individualized patient care, provide skilled nursing and symptom management, educate families, and collaborate with an interdisciplinary hospice team to support comfort and quality of life.


Primary Service Area

• Fort Worth and assigned surrounding communities

Local travel is required. The specific coverage territory will be determined according to patient census and agency needs.


Key Responsibilities

• Conduct comprehensive initial and ongoing nursing assessments.
• Develop, implement, and update individualized Plans of Care based on patients’ conditions, goals, needs, and end-of-life preferences.
• Provide skilled nursing care, comfort-focused interventions, and effective pain and symptom management.
• Monitor patient conditions and respond promptly to clinical changes or uncontrolled symptoms.
• Coordinate care with physicians, the Medical Director, social workers, chaplains, hospice aides, facility staff, and other interdisciplinary team members.
• Communicate changes in condition, caregiver concerns, medication issues, safety risks, or other significant findings to the appropriate team members.
• Educate patients and families about disease progression, medications, symptom management, medical equipment, comfort measures, and caregiving techniques.
• Provide compassionate guidance and emotional support to patients and families.
• Coordinate medications, supplies, equipment, and services in accordance with the patient’s Plan of Care.
• Maintain accurate, complete, and timely clinical documentation in the agency’s electronic medical record system.
• Participate in Interdisciplinary Group meetings and provide meaningful clinical updates regarding assigned patients.
• Participate in the on-call rotation as required.
• Ensure compliance with hospice regulations, professional standards, HIPAA requirements, and Millenia Hospice policies and procedures.
• Represent Millenia Hospice with professionalism, compassion, dignity, and respect.

Required Qualifications

• Current and unrestricted Registered Nurse license in the State of Texas.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and ability to travel throughout the assigned territory.
• Strong clinical assessment, care coordination, and critical-thinking skills.
• Excellent communication, organizational, and interpersonal abilities.
• Ability to work independently and manage a field-based patient caseload effectively.
• Ability to prioritize changing patient needs and respond appropriately to urgent concerns.
• Ability to complete accurate and timely clinical documentation.
• Ability to successfully complete required background checks, license verification, and onboarding requirements.

Preferred Experience

• One to two years of hospice, home health, palliative care, or related nursing experience is preferred.
• Experience with pain and symptom management, patient and family education, and interdisciplinary care coordination is highly valued.
