We are seeking an experienced and compassionate Full-Time Hospice RN Case Manager to provide exceptional, patient-centered end-of-life care throughout Dallas and surrounding communities. This field-based position is ideal for a dependable nurse who is committed to clinical excellence and helping patients experience comfort, dignity, and compassionate support wherever they call home.
As an RN Case Manager, you will coordinate each patient’s care, manage symptoms, educate families, and collaborate with our interdisciplinary team throughout the hospice journey.
Primary Service Area- This position serves patients in Dallas and surrounding communities.
Local travel is required. Patient assignments will be coordinated according to census and agency needs.
Key Responsibilities- Conduct comprehensive initial and ongoing nursing assessments.
- Develop and update individualized Plans of Care based on each patient’s condition, goals, needs, and end-of-life preferences.
- Provide skilled nursing care, comfort-focused interventions, and effective pain and symptom management.
- Monitor changes in patients’ conditions and promptly update the Plan of Care when necessary.
- Coordinate care with physicians, the Medical Director, social workers, chaplains, hospice aides, facility staff, and other interdisciplinary team members.
- Communicate significant changes in condition, uncontrolled symptoms, safety concerns, or caregiver needs to the appropriate clinical leader.
- Educate patients and families regarding disease progression, medications, symptom management, medical equipment, comfort measures, and caregiving techniques.
- Provide emotional support and guidance to patients and families throughout the hospice journey.
- Ensure medications, supplies, equipment, and other services are coordinated according to the Plan of Care.
- Maintain complete, accurate, and timely clinical documentation in the agency’s electronic medical record system.
- Participate in Interdisciplinary Group meetings and provide meaningful updates regarding assigned patients.
- Participate in the on-call rotation as required.
- Follow all hospice regulations, standards of practice, HIPAA requirements, and Millenia Hospice policies and procedures.
- Represent Millenia Hospice with compassion, professionalism, dignity, and respect.
Required Qualifications- Current and unrestricted Registered Nurse license in the State of Texas.
- Current CPR certification.
- Valid driver’s license, reliable transportation, and ability to travel throughout the assigned service area.
- Strong clinical assessment, care coordination, and critical-thinking skills.
- Excellent communication, organizational, and interpersonal abilities.
- Ability to work independently, prioritize patient needs, and manage time effectively.
- Ability to complete accurate and timely clinical documentation.
- Ability to successfully complete required background checks, license verification, and onboarding requirements.
Preferred Experience- One to two years of hospice, home health, palliative care, or related nursing experience is preferred.
- Previous experience with pain and symptom management, end-of-life education, or interdisciplinary care coordination is highly valued.
Compensation and Benefits- $90,000 annual compensation
