Position Summary
The Hospice/Home Health Social Worker provides emotional, social, and practical support to patients and families receiving hospice or home health services. This role is a key member of the interdisciplinary care team, ensuring patients receive holistic, patient-centered care throughout their healthcare journey.
Job Responsibilities- Conduct psychosocial assessments for hospice and home health patients
- Provide emotional support, counseling, and crisis intervention for patients and families
- Assist patients and caregivers with advance care planning and end-of-life support
- Connect patients with community resources and support services, including:
- Medicaid application assistance
- Meals on Wheels and nutrition programs
- Medical transportation services
- Caregiver support services
- Utility assistance and financial resources
- Housing assistance and placement support
- Coordinate respite care services and hospice transfers as needed
- Collaborate with nurses, physicians, chaplains, and interdisciplinary team members
- Advocate for patient rights, dignity, and quality of life
- Maintain accurate documentation in compliance with hospice and home health regulations
Education and Certification Requirements- Bachelors or Masters degree in Social Work from an accredited program
- Active Idaho Social Work license
- Licensed Master Social Worker (LMSW) required
- Experience in hospice, home health, medical social work, or healthcare setting preferred
- Knowledge of community resources, Medicaid, and social service systems
- Strong communication, organization, and time-management skills
- Valid drivers license with the ability to travel to patient homes
