We're looking to hire a Full-Time Registered Nurse (RN) Case Manager with compassion and experience to help us grow. This position will start with a very small case load within our primary service area, with a temporary focus to assist the DON with supervisory visits and initial assessments within our entire territory (up 35 miles from Plymouth).This is a traveling position with high autonomy! Ability to commit to at least 32-40 hours per week. No rotation on-call, no evenings and no weekends.  We promise a comfortable case load with flexible scheduling (make your own schedule) to help with work life balance.
