Independence Home Health LLC is looking for a dedicated and compassionate Full-Time RN Case Manager to join our growing team! Position Details:
- Full-time, Monday–Friday
- Report to the office at 8:00 AM to pick up the company vehicle for scheduled visits
- Complete patient visits and case management responsibilities as scheduled
- Work collaboratively with patients, families, caregivers, and the clinical team to ensure quality care and continuity of services
Qualifications:
- Current, valid Registered Nurse (RN) license
- Experience in home health preferred
- At least one to three years of case management experience preferred
- Strong communication, organizational, and critical-thinking skills
- Compassionate, dependable, and patient-focused
