Their providers bring quality medical care to the geriatric population, making a dramatic impact in increasing quality of life and safeguarding the dignity of those they serve.POSITION DESCRIPTION: Serve as a primary care provider for geriatric patients located in a skilled nursing facility. Overall duties of the role include, but are not limited to:- plan, direct, and evaluate medical care- examine and treat chronic illnesses- examine and treat urgent illnesses- order and interpret laboratory and diagnostic testing- collaborate with caregivers and other healthcare professionals- work closely with the interdisciplinary care team- make referrals as necessaryThe practice uses an EHR on a mobile-based platform and providers are supported by team members who assist with scheduling, orders, referrals, etc.Providers share on-call responsibilities, which is low volume phone triage.
