This role is ideal for an LPN who enjoys delivering personalized, one-on-one patient care in a home setting.
Position Overview
- Full-time, permanent opportunity

Job Responsibilities
- Deliver direct patient care in accordance with individualized care plans
- Monitor patient conditions and report changes to the RN or physician
- Administer medications and treatments as prescribed
- Perform wound care and other skilled nursing procedures
- Educate patients and families on health management and treatment plans
- Maintain accurate and timely documentation
- Ensure compliance with all healthcare regulations and agency standards
Education & Certification Requirements
- Graduate of an accredited Practical Nursing program
- Active LPN license in the state of Missouri
- Current BLS certification
- Minimum of 1 year of nursing experience preferred (home health experience a plus)
- Valid drivers license and reliable transportation required
