Job Description:
- Provide comprehensive social work services to patients and their families, including psychosocial assessments, counseling, resource referrals, and discharge planning.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Advocate for patients rights and coordinate community resources to ensure continuity of care.
- Maintain accurate and thorough documentation in compliance with regulatory standards and hospital policies.
- Participate in ongoing professional development and training opportunities.
Education and Certification Requirements:
- Masters degree in Social Work from an accredited institution.
- Current licensure as a Social Worker in the state of Arizona.
- Minimum of 2 years of clinical social work experience preferred.
- BLS certification required.
