Full-Time Hospice RN Case Manager

Connected Health Care is partnering with a respected healthcare organization to hire an experienced Hospice RN Case Manager in Boise, Idaho. This role is ideal for a compassionate Registered Nurse who is committed to providing high-quality, patient-centered end-of-life care.

Responsibilities

• Provide skilled, compassionate nursing care to hospice patients in their homes and community settings.
• Manage an assigned patient caseload and coordinate care with the interdisciplinary hospice team.
• Perform nursing assessments and develop and update individualized plans of care.
• Monitor patient conditions and communicate changes to physicians and the hospice medical director.
• Manage pain and symptoms while promoting comfort and dignity.
• Educate and support patients and families regarding medications, disease progression, and end-of-life care.
• Maintain accurate clinical documentation and participate in interdisciplinary care planning.
• Respond appropriately to patient and family needs while maintaining hospice standards and regulations.
• Travel throughout the Boise area for patient visits.

Requirements

• Current, unrestricted Idaho RN license or eligibility for Idaho licensure.
• ADN required; BSN preferred.
• Current BLS/CPR certification.
• Minimum of 1 year of Hospice RN Case Manager experience required
• Valid driver's license, reliable transportation, and required auto insurance.
• Strong clinical assessment, communication, documentation, and organizational skills.
• Ability to work independently and effectively as part of an interdisciplinary team.
• Compassionate, patient-centered approach to care.

Preferred Qualifications

• Hospice or palliative care experience.
• Previous RN Case Manager experience.
• Knowledge of Medicare hospice regulations.
• Experience with pain management, symptom control, and end-of-life care.
