Devoted Care Hospice is seeking a compassionate, experienced Registered Nurse Case Manager to join our growing clinical team in a full-time role.


We are looking for an RN who is confident working independently in the field, comfortable managing a hospice caseload, and committed to providing exceptional end-of-life care to patients and families.


Prior hospice experience is strongly preferred. The ideal candidate understands the unique responsibilities of hospice case management, including symptom management, decline recognition, care coordination, family education, and timely clinical documentation.


Position Overview


As a Full-Time Hospice RN Case Manager, you will oversee the nursing care and coordination of an assigned patient caseload. You will serve as a primary clinical resource for patients and families while collaborating closely with physicians, hospice providers, facilities, and members of the interdisciplinary team.


Patients may reside in private homes, assisted living communities, personal care homes, nursing facilities, or other residential settings throughout the service area.


Responsibilities


  • Manage an assigned hospice patient caseload and provide ongoing clinical oversight.
  • Complete comprehensive nursing assessments, routine visits, and PRN visits as clinically indicated.
  • Develop, implement, and update individualized plans of care.
  • Assess for changes in condition, disease progression, and evolving symptom-management needs.
  • Manage pain and other end-of-life symptoms using hospice best practices.
  • Communicate changes in condition and obtain appropriate orders from physicians and hospice providers.
  • Provide education, support, and guidance to patients, families, and caregivers.
  • Coordinate care with hospice aides, social workers, chaplains, physicians, facilities, pharmacies, DME providers, and other members of the interdisciplinary team.
  • Participate in interdisciplinary care planning and team meetings.
  • Complete accurate, thorough, and timely documentation in the electronic medical record.
  • Complete admissions, recertification assessments, deaths, and other clinical needs as appropriate.
  • Maintain communication with the clinical team regarding significant changes, concerns, and patient needs.
  • Ensure care is provided in accordance with the hospice plan of care and applicable regulatory requirements.
Qualifications


  • Current Texas Registered Nurse license in good standing.
  • Minimum one year of RN experience required.
  • Previous hospice RN or hospice case management experience strongly preferred.
  • Home health, palliative care, oncology, ICU, ER, geriatrics, or related experience may also be considered.
  • Strong clinical assessment and independent critical-thinking skills.
  • Ability to prioritize and manage a patient caseload effectively.
  • Strong organizational and time-management skills.
  • Excellent communication and compassionate bedside manner.
  • Comfortable communicating with physicians, facilities, patients, families, and interdisciplinary team members.
  • Reliable transportation, valid driver's license, and current automobile insurance.
  • Current CPR certification.
  • Experience with WellSky/Kinnser is a plus.
Schedule


  • Full-Time
  • Primarily weekday field-based patient care
  • Participation in clinical coverage and/or on-call rotation as scheduled
  • Schedule may vary based on patient needs
