We're seeking a skilled and empathetic Full Time Hospice Case Manager to join our dynamic team in the Flint and surrounding areas. This position offers a rewarding opportunity to make a significant impact in the lives of those we serve.
Key Responsibilities:
- Conduct initial and ongoing assessments of patients’ needs to develop comprehensive care plans.
- Provide emotional support and education to patients and families about end-of-life care and self-care techniques.
- Collaborate with the interdisciplinary team to ensure high-quality care and regular updates on patient progress.
- Supervise and mentor nursing staff and paraprofessionals in delivering exceptional care.
- Maintain accurate clinical documentation and participate in performance improvement initiatives.
Requirements
- Current Michigan Registered Nurse (RN) License in good standing.
- Minimum of 1 year experience in hospice or palliative care is a plus.
- Ability to work collaboratively with an interdisciplinary care team.
- Empathetic and compassionate approach with patients and families.
- Current driver's license.
- Eligible to work in the United States.
- Able to travel across Flint, Lapeer, and surrounding areas for home visits.
- Willingness to participate in weekend on-call duties on a rotating basis.
- Reliable personal vehicle for home visits across the surrounding areas.
