Full-Time Physical Therapist (PT) Near Kansas CIty, MO
Connected Health Care is seeking a dedicated and efficient Physical Therapist (PT) for a full-time opportunity in Kansas CIty, MO. This position is with one of our valued healthcare partners and offers the chance to work in a dynamic home health setting where your expertise directly improves patient outcomes and quality of life.
We are looking for a clinician who thrives in a fast-paced environment, is highly organized, and excels in patient-centered care. Experience with home health processes and documentation is strongly preferred.
Job Responsibilities
- Provide high-quality physical therapy services in a home health setting
- Perform patient evaluations and develop individualized treatment plans
- Complete Start of Care assessments and ongoing case management
- Conduct OASIS assessments accurately and efficiently
- Manage re-certifications and ensure timely documentation compliance
- Collaborate with interdisciplinary teams to coordinate patient care
- Educate patients and families on treatment plans and home exercises
- Maintain accurate and timely documentation using Matrix Care EMR
- Monitor patient progress and adjust treatment plans as needed
Education & Certification Requirements
- Doctor of Physical Therapy (DPT) or equivalent from an accredited program
- Active Physical Therapist license in Missouri and Kansas required
- Current CPR/BLS certification
- Minimum of 1 year of home health experience preferred
- Strong knowledge of OASIS, case management, and re-certification processes
- Proficiency with EMR systems with Matrix Care experience preferred
