Home Health Physical Therapist (PT) Job Responsibilities
- Perform comprehensive patient evaluations and develop individualized treatment plans
- Conduct start of care assessments and complete OASIS documentation accurately
- Provide routine home health therapy visits focused on improving strength, balance, mobility, and independence
- Complete patient recertifications and reassessments as required
- Educate patients and caregivers on therapeutic exercises, mobility strategies, and home safety
- Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams
- Monitor patient progress and modify treatment plans to maximize outcomes
- Maintain timely and accurate clinical documentation using Matrix EMR
- Deliver compassionate, patient-centered care in a home-based setting
Education & Certification Requirements
- Doctorate or Masters degree in Physical Therapy from an accredited program
- Active Missouri Physical Therapist (PT) license required
- Current CPR/BLS certification preferred
- Home health experience is not required
- Strong communication, organization, and time management skills
- Ability to work independently and manage a daily patient schedule
- Experience with OASIS documentation and Matrix EMR preferred but trainable
