NATURE OF WORK
This position serves as a Case Manager at the Family Peace Center (FPC). The Family Peace Case Manager (Case Manager) will assess the needs of clients and their family unit and arrange or provide a range of direct individual services to meet the immediate and long-term needs of clients.
GRANT FUNDING ADVISEMENT
This part-time position is 100% funded by a grant from the Department of Justice, Office on Violence Against Women. Acceptance of this position shall not be construed as a guarantee of employment beyond the grant-funded period.
DISTINGUISHING FEATURES
The Case Manager will receive referrals from the Family Peace Center Legal Advocate (Legal Advocate) and other criminal justice and court-related personnel. The Case Manager will provide direct, survivor-centered case management services. This includes offering emotional support, safety planning, and guidance tailored to the unique needs of each survivor. The Case Manager will conduct thorough needs assessments and develop individualized support and safety plans designed to increase survivors' long-term safety and stability.
A central part of this role is connecting survivors to FPC's network of over 30 partner agencies to access services such as housing, counseling, mental health care, child care, health care, employment assistance, and other stabilizing resources. The Case Manager will also support survivors in building relationships and skills necessary for healing and self-sufficiency, while helping them navigate complex systems, including legal proceedings, and long-term service planning.
ESSENTIAL FUNCTIONS (These essential duties are only illustrative.)
Complete grant goals and objectives according to proposed timelines, including: providing long term case management to FPC clients while adhering to survivor centered principles and policies of the FPC; treating all clients, staff and FPC partner agency personnel with kindness, dignity and respect; engaging in activities and practices addressing self-care and vicarious trauma; ensure the confidentiality and information sharing process is followed; participate in training and regular debriefs with the FPC Administrative Staff; provide feedback to the FPC Executive Director about additional services, partners or people who need to be at the FPC in order to fulfill client multi-victimization needs. The Case Manager will work with clients to develop and maintain relevant and meaningful case empowerment plans, which include addressing employment, wellness, health, housing advocacy, counseling, and court accompaniment support. The Case Manager will meet regularly with clients to support plan accomplishment, refer clients to other agencies and resources, advocate directly with other agencies as needed to help clients access necessary services, and accompany clients to services and appointments to support the successful implementation of the case management plan. The Case Manager will also ensure clients have an understanding and knowledge of resource options and facilitate connections and enrollment in those resources, and mediate on behalf of clients when appropriate.
Complete all other tasks and duties as assigned and required by the Executive and Deputy Director of the Mayor's Office of Domestic and Community Violence Prevention and the FPC Project Manager.
SUPERVISION RECEIVED
Works under the overall supervision of the FPC Project Manager in general and day-to-day supervision, who reviews work for effectiveness, timeliness, and overall results achieved.
SUPERVISION EXERCISED
This position does not require supervisory duties.
WORKING CONDITIONS & PHYSICAL DEMANDS
Work is performed primarily in a variety of settings, including the office and various locations, to meet clients needs of support and advocacy. Work activities occur in a fast-paced, high-demand environment at times.
SUCCESS FACTORS (KSAs)
Thorough knowledge of domestic violence and human trafficking prevention efforts, including best practice prevention efforts and strategies.
Comfortable working in a collaborative manner in a fast-paced environment with multiple organizations. Sensitivity to the concerns of partner organizations, willingness to receive and use feedback in program design, interpersonal interactions, and policy development.
Ability to express ideas effectively orally and in writing, to conduct research, and to prepare clear and concise reports.
Ability to establish and maintain effective working relationships with city employees, other government agencies, law enforcement, the general public, and community organizations.

EDUCATION, TRAINING & EXPERIENCE
Graduation from an accredited college or university with a Bachelor's Degree in human services, social services or the equivalent, and at least one year of experience providing case management or advocacy to persons experiencing domestic violence, dating violence, sexual assault, and/or stalking, 40-hour ICADV Domestic Violence Training and other training in confidentiality and mandated reporting and have knowledge about the Family Justice Center movement and the purpose of this model and service delivery. Knowledge of best practice interventions for persons in trauma from domestic violence, dating violence, sexual assault, and stalking. Experience with computer programs, data collection, and grant reporting is desired. Any satisfactory equivalent combination of education, experience and training which ensures the ability to perform the work may be substituted for the required experience.
NECESSARY SPECIAL REQUIREMENTS
Possession of a valid Illinois driver's license.
40-hour Illinois Domestic Violence training (may be completed within 30 days of hire).
Residency requirement:
All employees (except Police and Fire) shall reside anywhere in Winnebago County or anywhere within fifteen (15) miles of the Rockford City Hall within six (6) months of completion of their introductory period, department heads and City Administrators shall live within the municipal boundaries of the City of Rockford within six (6) months of their completion of their introductory period.
