Home visits in assigned service area: Largo, St. Petersburg, Pinellas Park, Seminole, and surrounding areas.
The Home Health Field Physical Therapist provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols.
Responsibilities include:
- Evaluates and develops specific treatment programs for individual patients based on their needs and in accordance with the principles and practices of physical therapy.
- Responsible for providing the appropriate level of clinical direction for Physical Therapy Assistants.
- Plans, implements, manages, and evaluates the provision of both professional and ancillary home health services, direct care, and case management to ensure that all patient needs are met and quality care is provided in accordance with Federal, State, JCAHO, and program guidelines.
- Manages the activities of the multi-disciplinary team providing care to the patient.
- Monitors the provision of therapeutic services through written and verbal communication with all disciplines.
- Develops, implements, and carries out a discharge plan in conjunction with the patient, family/caregiver, and members of the health care team.
- Interprets agency policy to patients/families.
- Reviews patients visit weekly and provides plan to Home Care Supervisor and scheduler, including anticipated weekend visits.
- Informs scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor.
Home Health Physical Therapist Requirements:
- Bachelors Degree in Physical Therapy
- Physical Therapist State of Florida License
- 1-year Physical Therapist experience
- Basic Life Support
- Drivers License (Florida drivers license is required within 30 days of hire)
Preferred:
- Masters Degree in Physical Therapy
