    Position Summary
    A well-established, not-for-profit community hospital is seeking a Family Nurse Practitioner (FNP) to join its outpatient primary care practice in Southbury, Connecticut. Candidates with a strong background in primary care, chronic disease management, and preventive medicine will be considered.

    This is a full-time, permanent outpatient-focused position providing comprehensive care to a diverse patient population across the lifespan, with the opportunity to manage a wide range of conditions including diabetes, hypertension, asthma, and other chronic and acute illnesses. Both experienced nurse practitioners and new graduates are encouraged to apply.

    Essential Responsibilities
    Provide comprehensive primary care services, including evaluation, diagnosis, and treatment

    Conduct routine physical exams, preventive screenings, and health assessments

    Manage acute and chronic conditions such as diabetes, hypertension, asthma, and infections

    Order and interpret diagnostic tests including labs and imaging

    Prescribe medications and develop patient-centered treatment plans

    Educate patients on disease prevention, lifestyle changes, and overall wellness

    Collaborate with physicians and multidisciplinary teams for coordinated care

    Maintain accurate and timely EMR documentation

    Participate in quality improvement and patient care initiatives

    Qualifications
    Education: Master’s or Doctorate degree in Nursing from an accredited program

    Certification: Board Certified as a Family Nurse Practitioner

    Licensure: Active Connecticut APRN license (or eligibility)

    Experience: Open to both experienced providers and new graduates

    Strong communication and clinical decision-making skills
