Title: Family Nurse Practitioner- PRN (As Needed)

Location: Indian Health Clinic- 703 NE Hancock St., Portland, OR 97212 & Wellness Center- 12360 E. Burnside St., Portland, OR 97233

Schedule: This is an in-person PRN / as-needed position, Monday through Friday. Hours are flexible and can vary between 0 - 40 per week depending on availability and clinic needs.

Wage Range: $75.00 - $85.00 hourly, Non-Exempt

Position Overview:

The PRN Family Nurse Practitioner (FNP) serves as a provider of primary health care services, coordinating and referring clients for health care and health education while delivering comprehensive, patient-centered care. This position provides clinical coverage on an as-needed basis and supports continuity of care in accordance with clinic needs, professional standards, and applicable licensing requirements.

This position functions in a PRN (as-needed) capacity, with scheduling and hours determined by clinic needs and patient care demands.

What you will do:

  • Provide comprehensive primary health care services on an as-needed basis to support clinic operations, including provider absences and fluctuations in patient demand.
  • Coordinate patient care with other providers and agencies and advocate for clients within health care systems.
  • Provide services in prenatal integrated health care of high-risk chemically dependent pregnant patients, in consultation and/or collaboration with obstetrical health care providers.
  • Perform initial and ongoing health risk assessments and counseling for high-risk lifestyle behaviors and/or familial diseases, including ongoing drug and alcohol assessment and HIV screening.
  • Encourage family participation in the health care of patients as deemed appropriate by the patient and other clinic providers.
  • Develop and document plans of care and outcomes evaluations.
  • Coordinate with contractual pharmacy, primary care providers, hospitals, and laboratories for required diagnostic procedures and delivery/births.
  • Provide health education to individual patients as needed, including birth control, safer sex, high-risk behaviors, and associated health consequences.
  • Coordinate the care patients receive from other providers and agencies and make referrals as appropriate.
  • Engage in ongoing education and specialized training in order to maintain a high quality of care to the Clinic's patient population.
  • Attend meetings and participate in clinic activities as requested, as needed.
  • Document patient care accurately and appropriately in accordance with organizational policies and professional standards.
