    Essential Responsibilities

    Provide comprehensive outpatient family medicine care to patients across the lifespan

    Evaluate, diagnose, and treat acute and chronic medical conditions

    Provide preventive care, routine health screenings, and wellness services

    Manage chronic conditions such as diabetes, hypertension, hyperlipidemia, and cardiovascular disease

    Perform routine physical examinations and coordinate appropriate diagnostic testing

    Develop individualized treatment plans and provide patient education on health, wellness, and disease prevention

    Collaborate with specialists, advanced practice providers, and multidisciplinary care teams to ensure coordinated patient care

    Maintain accurate and timely documentation within the EMR system

    Participate in quality improvement initiatives and clinical program development

    Qualifications

    Education: MD or DO from an accredited medical school

    Training: Completion of an accredited Family Medicine Residency

    Licensure: Active Connecticut medical license (or eligibility)

    Certification: Board Certified/Board Eligible in Family Medicine

    Experience: Open to both experienced Family Medicine Physicians and graduating residents

    Compensation & Work Environment
