Family Medicine Physician for Innovative Population Health Clinic

The University of Utah Department of Family and Community Medicine, within the Department of Family Medicine and Public Health, is seeking a dedicated board-certified family medicine physician to join our dynamic team in the position of Family Medicine Physician Clinical Faculty. This unique position combines clinical care, teaching, and scholarship within the University of Utah Family Medicine Residency Program and the Division of Family and Community Medicine, while also providing innovative, patient centered care in our Intensive Outpatient Clinic. The ideal candidate will be passionate about advancing academic medicine, mentoring future family physicians, and innovating in population health and value based care spaces. We are seeking a candidate with the vision and skills to grow into a leadership role and further develop, implement, and advance our value-based care initiatives and programs.

The Department of Family Medicine and Public Health is comprised of 2 divisions: Family and Community Medicine and Public Health. This structure provides a rich environment for scholarship and collaboration. You will have access to research support from pre-award to dissemination support. The division of Family and Community Medicine includes primary care and urgent care services, includes11 clinics, spans 5 counties, employs over 200 clinicians and serves more than 574,600 patients annually.

This position's clinical practice location is an innovative multi-disciplinary clinic. The Intensive Outpatient Clinic offers a collaborative, multidisciplinary care model for complex patients, with longer visit times and embedded case management, behavioral health, addiction medicine, oral health, and social support-so physicians practice as part of a true team rather than managing high-need patients alone. Providing comprehensive, integrated, and innovative care, the IOC aims to improve clinical outcomes and decrease the overall cost of care for the identified population. In 2023, Utah Governor Cox's One Utah Health Collaborative recognized the IOC has as leader in novel comprehensive care delivery and sustainable financial innovation.

The IOC was also featured in Meet Me Where I Am - a short film that premiered in the 2022 Sundance Film Festival - that tells the story of one IOC patient and how this clinic provides unique integrated care. The IOC exclusively serves a Medicaid population and operates under an alternative payment model that rewards quality, risk adjustment, care coordination, and a bonus for reducing ER visits and hospitalizations. This position reports to the IOC Medical Director and will support the IOC patient population and the clinic team located at the University of Utah Population Health Center Fair Park (north Salt Lake). Clinical activities include but are not limited to: Ambulatory Office Visits with History & Physical Exam review, Telehealth Visits, Telephonic Visits, Supporting Care Coordination, management of complex mental health and substance use disorders, interdisciplinary meetings and care planning, and Medication Management. The ideal candidate will also have a working knowledge of trauma informed care, mental health, and substance use disorders care.

Qualifications

  • Board certification by The American Board of Family Medicine.
  • Unrestricted Utah state medical license.
  • Experience in clinical medicine, teaching, and scholarship is preferred.

Responsibilities

  • 0.90-0.95 FTE Clinical care at the Intensive Outpatient Clinic
  • 0.05 -0.10 FTE Scholarship: Scholarly projects, research and administration
  • 1.0 FTE Total
