What You’ll Do
- Provide outpatient geriatric care for adults over age 55 within the PACE program
- Conduct off-site patient visits at residences, assisted living, or SNFs
- Collaborate with an interdisciplinary team of 11 healthcare professionals
- Manage patient cases primarily via EMR systems
- Participate in orientation and ongoing clinic duties
- Contribute to patient care coordination in the community
- Ensure compliance with all licensing and certification requirements
What You Need
- Required:
- Active Oregon medical license and DEA
- Board Certified or truly Board Eligible in Internal Medicine or Family Medicine
- BLS certification
- Geriatric care experience preferred
- Preferred:
- Experience working with senior populations and in community settings
- Ability to work 3-4 days per week
