Southwest Center is seeking compassionate and dependable individuals interested in becoming Family Home Providers (FHPs). Family Home Providers offer support in a home-based setting for individuals with intellectual and developmental disabilities while promoting independence, dignity, safety, and meaningful community involvement.

This is a contract provider opportunity that requires successful completion of Southwest Center’s provider approval and certification process before services may begin.

About the Role
Family Home Providers create a supportive and welcoming environment for individuals receiving services. Providers may assist with daily living support, transportation, community participation, personal growth goals, and maintaining healthy relationships with family and support teams.

Providers are expected to:
  • Maintain a safe and respectful home environment
  • Support person-centered goals and choices
  • Encourage community inclusion and independence
  • Work collaboratively with Southwest Center and the individual’s support team
  • Maintain professional communication regarding services and supports
 
Provider Approval & Home Assessment Process
Before approval, Southwest Center completes a home assessment and provider review process. This process may include multiple meetings or home visits to ensure the home environment and household are appropriate for services.

Areas reviewed during the process may include:
  • Household readiness and willingness to welcome an individual into the home
  • Ability to provide a stable, supportive, and respectful environment
  • General health and safety of the home
  • Cleanliness and housekeeping standards
  • Access to community resources and activities
  • Transportation availability
  • Ability to support ongoing relationships with family, friends, and care teams
  • Understanding of provider responsibilities and agency expectations
  • Willingness to participate in training, communication, and ongoing oversight
Each home is evaluated individually to determine whether it is an appropriate match based on the needs, preferences, and supports required by the individual receiving services.

Required Documentation & Screening Items
Applicants should be prepared to complete and provide the following during the approval process:
  • Provider application and onboarding paperwork
  • Verification of identity and work authorization documentation
  • Physician statement or medical verification forms
  • TB risk assessment and any required follow-up testing
  • Criminal background checks for applicable household members
  • Driver’s license and vehicle insurance documentation for household drivers
  • Drug screening
  • Additional screening or driving record reviews if applicable
Southwest Center staff will assist providers throughout the onboarding, certification, and required training process.
