    We are looking to hire a compassionate and patient-centered Family Practice Advanced Practice Provider for their Primary Care department.

    Reporting to the Chief Medical Officer, The Family Practice Nurse Practitioner/Physician Assistant provides comprehensive primary care services in accordance with established medical protocols and standards of practice

    Schedule: Full-Time | 1.0 FTE

    What You’ll Do

    Key Responsibilities

    Under the supervision of the supervising or collaborating Physician, the Family Nurse Practitioner/Physician Assistant will:

    • Record medical histories, perform physical exams, provide follow-up care and individualized patient instruction according to treatment plans and medical protocols
    • Prescribe appropriate medications according to medical protocols and standard medical practice
    • Provide medical services within the scope of practice established by licensure and/or certification, including additional services such as MAT or PrEP as determined by the Chief Medical Officer
    • Complete accurate clinical documentation according to clinic protocols, funding requirements, and standard medical practice
    • Identify physical, nutritional, psychosocial, emotional and/or lifestyle concerns and refer patients to appropriate on-site or off-site resources
    • Maintain compliance with clinical and administrative quality measures, including HEDIS metrics and patient productivity
    • Consult with the supervising/collaborating physician for patient evaluations as needed
    • Complete timely EHR documentation and actions, including diagnoses, treatment plans, orders, and medications
    • Address pending EHR PAQ items in a timely manner
    • Respond promptly to E-Consult specialist reviewers
    • Provide medical services through the EVCHC telehealth platform
    • Adhere to QI guidelines and participate in chronic disease Quality Improvement programs
    • Collaborate with Registered Dietitians and Health Educators to provide appropriate patient education
    • Participate in monthly Provider Meetings as required
    • Serve as a preceptor for Nurse Practitioners, Physician Assistants, or medical students as assigned
    • Attend conferences and seminars as requested and fulfill continuing education requirements
    • Participate in provider recruitment activities
    • Participate in activities that support the development of clinical support team members

    What We Are Looking For

    Education, Licenses and Certifications

    • Completion of an accredited Advanced Practice Provider training program
    • Current California Nurse Practitioner or Physician Assistant license
    • Current DEA Certification + Current CPR Certification
    • Current Board Certification from an accredited APP certifying organization

    Additional Skills and Attributes

    • Ability to work effectively with diverse cultural, ethnic, and socioeconomic groups within the service area
    • Ability to work independently with minimal supervision
    • Ability to communicate clearly through speaking, hearing, and writing
    • Ability to write notes, treatment plan comments, track records and reports
    • Ability to work at a computer for extended periods
    • Positive and professional communication with staff at all levels
    • Ability to stand, sit, stoop, kneel, and bend in order to speak to patients

    • Primarily a clinical site with a small office space shared with multiple employees
