Qualification and CompetenciesHigh School Diploma preferred. At least 3 years experience as a mental health recipient, who has successfully utilized opportunities for personal growth. Ability to respond appropriately to supervision and work as a team member. Certified by the Academy of Peer Services or undergoing certification is preferred. General DescriptionWill act as a recovery coach to assist clients in working toward stability. Will utilize past experience and success as a recipient in order to serve as a role model to Supported Housing clients.  Duties and ResponsibilitiesSupport clients' community integration and socialization by connecting them with appropriate community resources and activities.Assist clients and partner housing agencies in accessing outside supports, including Wrap-Around services through the Nassau County Office of Mental Health.Provide supportive counseling and education on medication and symptom management, coping strategies, and recovery.Work with clients to develop Wellness Recovery Action Plans (WRAP) and provide follow-up after discharge.Coordinate services by maintaining communication with treatment teams, Health Home care coordinators, and housing staff.Participate in supervision, staff development, and work-related workshops or conferences as scheduling permits.Respond appropriately to emergencies, complete required documentation and reports on time, and perform other assigned duties. Location:Office: 1976 Grand Ave, Baldwin, NY 11510Field: Nassau County, NY
