    Description

    Overview:The Home Health RN delivers compassionate, patient-centered nursing care in a home environment. This role focuses on helping patients recover, manage chronic conditions, and maintain independence while ensuring high-quality clinical outcomes.

    Primary Duties:

    • Conduct comprehensive in-home health assessments and ongoing evaluations

    • Create and manage personalized care plans in collaboration with physicians

    • Provide skilled nursing services including injections, IV therapy, catheter care, and wound management

    • Track patient conditions and adjust care plans as needed

    • Maintain detailed and timely clinical documentation

    • Teach patients and caregivers about treatments, medications, and preventive care

    • Communicate regularly with healthcare providers and interdisciplinary team members

    • Follow infection control practices and ensure patient safety at all times

    Required Qualifications:

    • Current RN license in good standing

    • Degree from an accredited nursing program

    • Valid CPR/BLS certification

    • At least 1 year of nursing experience (home health or community care preferred)

    • Excellent clinical judgment and problem-solving abilities

    • Strong interpersonal and communication skills

    • Ability to travel to patient homes and work independently

    Work Setting:

    • Care is provided in private residences or assisted living settings

    • Frequent local travel required

    • Flexible work hours based on patient needs

    Physical Demands:

    • Ability to assist patients with mobility and daily care needs

    • Frequent standing, bending, and lifting

    • Driving between patient visits
